Job Description
Role Overview
The successful candidate will undertake Continuing Healthcare (CHC) and Funded Nursing Care (FNC) assessments for adults with complex health needs. This will include face-to-face assessments across the Birmingham and Solihull areas, with report writing completed either from home or the office.
Key Responsibilities
- Complete CHC and FNC assessments for adults with complex health needs
- Undertake face-to-face assessments within Birmingham and Solihull
- Complete DSTs, Checklists, Fast Track assessments and reviews
- Work with patients, families, carers, advocates and multidisciplinary teams
- Prepare clear, accurate and comprehensive assessment reports
- Support CHC review panels and funding decision-making processes
- Manage a complex clinical caseload independently
- Liaise with care homes, local authorities, acute trusts and community services
- Ensure assessments are completed in line with the National Framework for NHS Continuing Healthcare and Funded Nursing Care
Essential Requirements
- Registered Nurse with current NMC registration
- Strong experience in CHC / Continuing Healthcare assessments
- Experience completing DST, Checklist, Fast Track and FNC assessments
- Good understanding of the National Framework for NHS Continuing Healthcare and Funded Nursing Care
- Experience working with adults with complex needs, long-term conditions, disabilities, palliative care or end-of-life care
- Excellent report writing and communication skills
- Ability to work independently and manage a complex caseload
- Ability to travel across Birmingham and Solihull for face-to-face assessments
Desirable Requirements
- Previous experience working within an ICB, CCG or NHS CHC team
- Experience presenting cases to CHC panels
- Knowledge of safeguarding, DoLS, Care Act and Mental Capacity Act
- Strong stakeholder management and negotiation skills
